1. 3 photocopies of PSA Birth Certificate
2. Original copy of Form 138-B (Report Card)
3. Original copy of Permanent Record (Form 137-A)
4. Photocopy of authenticated Marriage Certificate (if married)
5. Certificate of Good Moral Character
6. College Admission Test Results
7. Certificate of Residency
8. Interview by the Dean
9. 2 pieces 2x2 picture (white background with name tag)

ENROLLMENT Requirements (OLD STUDENT)

1. Complete signed clearance
2. 2 pieces 2x2 picture (white background with name tag)

ENROLLMENT Requirements (Transferees)

1. 2 pieces 2x2 picture (white background with name tag)
2. 3 photocopies of PSA Birth Certificate
3. Honorable Dismissal
4. Transcript of Records
5. Certificate of Good Moral Character
6. An interview from the Dean

ENROLLMENT Requirements (Returnees)

1. Apply for Return to Study to the College.
2. Attach a copy of the Student's Evaluation Record secured at the Office of the Registrar to the Application Form for Returning Student.
3. Complete Signed Clearance
