- TRANSFEREES
- Form 138 – (Report Card) w/ 3rd grading grade
- PSA Birth Certificate (Xerox Copy)
- Certificate of Good Moral Character
- Certificate of Eligibility to transfer

- Wait for the confirmation of your registration via EMAIL

FOR NEW STUDENT – Proceed to STEP 2 up to STEP 7
FOR OLD STUDENT – Proceed to STEP 2 on JUNE 1, 2022 once your Form 138 (card) was released

STEP 2:
- Go to the REGISTRAR’S OFFICE and submit the REQUIRED DOCUMENTS.
- FOR Grade 7 – Accomplish ESC Form

STEP 3:
- Proceed to the BUSINESS OFFICE for PAYMENT

STEP 4:
- Proceed to the LIBRARY to claim your BOOKS
|
STEP 5:
- Wait for you REGISTRATION FORM with a stamp ENROLLED
|
STEP 6:
- Go to the MIS office for your Office365 Username and Password
- For Grade 7 – Submit the accomplished ESC Form
- For Transferee – Submit the ESC Certificate

STEP 7:
- OFFICIAL ENROLLMENT LIST will be posted/view in our WEBSITE.
